Advance Request for Analytical Services
This form is to be used by USGS sample submitters for requesting analyses of samples for projects that fall late in the fiscal year and can not be completed by the end of year close out. All samples must be received by the GW Dating Lab by December 31 of the applicable calendar year. After receipt of the form, an SV will be prepared by the GW Dating Lab for funds transfer.
Please download the document, fill in the requested information, and email this document as an attachment to cfc@usgs.gov to have an SV issued.
