Sample Submission Form
All samples submitted must be accompanied by a submission sheet. It's in Microsoft Excel format. The sample submission spreadsheet is a means by which we can keep your samples sorted and your data accurate. Please download the newest version, v3.12, and submit it with your samples. Be cautious when copying and pasting. Pasting as values works the best. When shipping samples, Email the submission sheet to cfc@usgs.gov. Please let us know if there are problems with the submission sheet.
