New Analytical Services Request Form

In July 2000, the Office of Water Quality announced that a standardized 
Analytical Services Request (ASR) form would be implemented bureau wide, 
effective October 1, 2000. As of now, approximately 95% of all samples are 
being submitted to the National Water Quality Laboratory (NWQL) with the 
new ASR. 

According to the implementation schedule it was determined that any sample 
received after January 1, 2001 without the standardized ASR would be 
assessed a $20.00 processing fee, per Office of Water Quality Tech Memo 
2000.09. 

The new ASR is available on the NWQL Homepage at the following address in 
PDF format, FrameMaker versions 4 and 5, and Microsoft Word.

The ASR paper form is also available on 1stop shopping from the NWQL at a 
charge of $29.00 per pad.
