OPAC Billing Request Form (Form 9-2048)
To: "AO - All Administrative Officers"Attachment - FormFrom: "Tammy D'Elia" Subject: New OPAC form and procedures Administrative Officers: FYI, OFM has created a new OPAC Billing Request Form (Form 9-2048) with new fields required by Treasury. We need to start using the new form on December 1, 1999. You can access the new form and the instructions by going to https://interactive.usgs.gov/eft/logon.asp This will take you to the USGS Web Applications System where you will need to logon. Follow the instructions on that page. After you have logged on you will go to the web site where you can access "EFT for Travel" or "Forms". Select "Forms". When you get to the Forms page, please read the instructions there. You will need both Adobe Acrobat Reader 4.0 and Microsoft Internet Explorer 5 (MSIE5) to access this form. If you do not have both of these applications installed on your computer, the instructions on this page explain how to download both. You may need your Systems Administrator to help. After Adobe and MSIE5 are installed, you will be able to go to the "U.S. Geological Survey Forms" section and select *OPAC Billing Request (9-2048 November 1999). Then click on "Retrieve USGS Form". The first 3 pages of the document contain the instructions on filling out the form. The 4th page is the actual form. You can type the information directly on the form and print it out. I know this is short notice, so if you need any help, please call me. I was able to access the form today and it seems to work well. For those of you wondering why we will not be using Framemaker for the new OPAC form: Since USGS will soon be using Lotus Notes, we need to use to a new forms solution other than Framemaker that will work with Lotus Notes. It would be a waste of time to modify the template in Framemaker and then not be able to use it. Anyone who is able to access an Microsoft Excel file or would like to see how the form looks in Excel, I have attached file "opac Form 9-2048.xls". An OPAC web page has been also established which provides information on the new OPAC procedures.
