Sample Submission Form
The sample submission spreadsheet is a means by which we can keep your samples sorted and your data accurate. Please download the new version, v3.08, and submit it with your samples. Be cautious when copying and pasting.
Spreadsheet Version
The current spreadsheet is version 3.08. Any samples submitted with the wrong version or incorrect / incomplete spreadsheets will not be processed. This spreadsheet MUST be sent with your samples.
The Spreadsheet is provided only in MS Excel format.
Newer Excel Versions
You will need to ensure that you send us back this document in .xls format, not the newer .xlsx format. To do so, click "Save As..." and choose "Excel Workbook (*.xls)" from the "Save as type" dropdown menu. Our lab management system works with MS Excel 2003.
